Reference page for the 2022 recruitment | Reviewed: 14 October 2026

This URL was previously linked from the Directorate of Health Services, Assam website for Grade III and Grade IV applications.

The current SMB Form website does not operate the former application service. You cannot submit an application, download an old admit card or access your previous account here. This page explains the earlier recruitment and directs candidates to the departments responsible for official updates.

What Was the DME Assam AYUSH 2022 Application?

The official addendum advertisement described a common application form for Grade III Technical, Grade III Non-Technical and Grade IV posts under four Assam directorates:

  • Directorate of Medical Education—DME.
  • Directorate of Health Services—DHS.
  • Directorate of Health Services, Family Welfare—DHSFW.
  • Directorate of AYUSH.

Candidates could apply for available posts for which they met the qualifications stated in the respective advertisements. The notice also explained that candidates did not need to submit the same common application again through another department’s website.

Why Did an Official DHS Link Bring Me Here?

The DHS page titled “Online Application form for the post of Grade III and Grade IV” contains a “Click here to apply” link pointing to this old SMB Form URL.

That link refers to the former application service. Its presence on an official page does not mean that the current SMB Form website accepts government recruitment applications.

Where Should Candidates Check Current Information?

Use the website of the department named in your recruitment advertisement.

DepartmentOfficial website
Directorate of Medical Educationdme.assam.gov.in
Directorate of Health Servicesdhs.assam.gov.in
Directorate of Health Services, Family Welfaredhsfw.assam.gov.in
Directorate of AYUSHayush.assam.gov.in

Check the recruitment or notification section. Match the department, advertisement number, post name and recruitment year before following an application, admit card or result link.

An old advertisement remaining online does not establish that applications are currently open.

Help With an Old Application, Admit Card or Payment

If you applied through the former portal, contact the relevant department using the contact details in its official recruitment notice.

Keep your application acknowledgement, advertisement number, post name and payment receipt available for reference. Share personal information only through the department’s verified support channel when requested.

SMB Form cannot recover an old password, check an application status, issue an admit card or process a recruitment fee refund.
